ABOUT OUR CLINIC…..
We provide Occupational Medicine Services, which includes employee physicals, drug screens, and the care of workman’s compensation covered injuries.
We take walk-in patients for Urgent Care problems such as unexpected injury or illness, in those patients above the age six months.
We also provide Primary Care medicine for those age twelve and above. This would include the management of chronic medical problems such as heart disease, diabetes, hypertension, and many others, as well as providing routine health checkups.
Evergreen Clinic has on site X-ray services and can also perform many laboratory tests on-site.
We open early, and stay open late, and we are also open Saturdays for walk-in patients.
We cannot predict or promise how long you will have to wait to be seen. We chose it to be this way. We understand that taking walk-ins and having extended hours were services that the community needed and wanted. The cost of doing this, to you, the patient, is extended wait times when we are busy, which is often.
IF YOU ARE INTOLERANT OF WAIT TIMES WE WOULD ASK THAT YOU CHOOSE A DIFFERENT PRACTICE FOR YOUR MEDICAL NEEDS.
Patients are not brought back in the order in which they present. When you get brought back may depend on several factors; how busy we are, which provider will likely see you, and which particular room that you may need to be in based upon your chief complaint.
We request that you submit Prescriptions Refill Requests at least two weeks prior to when you will run out of your medication.
If you call with a question or problem that you need to talk to a provider about, the message will be placed in the Provider’s box and they will try to call you back by, or at the end of their next day in the office.
If you can’t wait that long you should come in to be seen by any Provider as a walk-in patient. If you do come in as a walk-in patient please expect to be seen by any provider, and not by the one you request. You will need a scheduled appointment to request to be seen by a certain provider.
Lastly, we do not admit patients to the hospital. If you are an established primary care patient at Evergreen and you will need to be hospitalized you will be admitted under a physician who does not practice at Evergreen. You may return to the clinic for follow up after you are sent home from the hospital.
We will try hard to see that you are pleased with our clinic, our staff, our providers, and the services that we offer.
